Patient Rights & Responsibilities
Patients are treated with respect, consideration and dignity.
- The patient has the right to personal privacy at check in and in evaluation and treatment areas.
- Patients are provided appropriate privacy.
- Interpretation services are available.
- To the degree known, complete information concerning their diagnosis, evaluation, treatment, and prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.
Prior to receiving care, patients are informed of their rights.
- Patients and staff are informed of patient rights.
- Patient and staff are informed of how to voice grievances regarding treatment.
- Report grievances to the Director of the Student Health Service, Patricia Dirk at (701) 231-8758 or Patricia.Dirk@ndsu.edu which are reviewed in a timely manner.
- If the grievance is unresolved at this level, students may be complete the "Report a Concern or Complaint" form located on the Student Affairs and Enrollment Management webpage or by calling (701) 231-7701.
- Patients and staff are informed of methods for providing feedback, including complaints.
- Patients may provide feedback by completing the online Student Feedback Form, paper suggestion form in the Student Health Service lobby, post appointment survey after a provider visit and/or the annual student feedback survey.
- Patients and staff are informed of that patient's right to change providers if other qualified providers are available.
- Patients and staff are informed about advance directives, as required by prevailing laws and regulations.
Prior to receiving care, patients are informed of their responsibility to:
- Provide complete and accurate information to the best of their ability about their health, any medications, including over-the-counter products and dietary supplements, and any allergies or sensitivities.
- Follow the agreed-upon treatment plan prescribed by their provider and participate in their care.
- Provide a responsible adult to provide transportation home and to remain with them as directed by the provider or as indicated on discharge instructions.
- Accept personal financial responsibility for any charges not covered by insurance.
- Behave respectfully toward all health care professionals and staff, as well as other patients and visitors.
