Understanding Your Needs and Developing a Plan
Our goal is to understand how your disability affects you in daily life, so we can work together to create an effective accommodation plan that gives you equal access to the University. Your experiences, personal history, and perspective are important to us, and we encourage you to share as much as you feel comfortable. Any documentation or information you can provide will help guide this process while we complete our interactive intake meeting.
Examples of Helpful Documentation or Information:
- Educational records (e.g., IEP, 504 Plan)
- Neuropsychological or educational evaluations
- Statements from healthcare, mental health, or service providers

- Audiology reports
- Vision assessments
- Vocational assessments
- Proof of accommodations used on standardized tests (e.g., SAT, ACT)
- SDS Disability Assessment Form (see below for the appropriate version)
