Graduate Student Forms

All forms listed below are electronic DocuSign forms. To initiate a form:

  • Click the form name and fill out the PowerForm Signer Information sheet. Required signers are marked with a red asterisk (*). If you're unsure who to list, contact your advisor or department.
    • Double-check all email addresses. The form won't be processed correctly if an email is wrong.
    • The "Change to Supervisory Committee" form has specific instructions. Click the sample form for details.
  • Click "BEGIN SIGNING." On the next screen, check the box and click "CONTINUE."
  • After completing the form, click "FINISH." Soon after you will receive and email with a link to the form, which you may use to track the status of the form.
  • You'll get a confirmation email once everyone has signed the document and it has been received by the Graduate School.
