As needed, the Customer Account Services department will provide students with customized documents on NDSU letterhead to meet their individual needs. If you are seeking a customized document for tax purposes, please see the 1098-T tax form.
Students utilizing military benefits should refer to the Finances section in Campus Connection and print the Account Summary document for reimbursement submissions.
Process:
- Review the sample letters below to determine which most closely meets your individual needs.
- Submit a request for a customized letter using the following link: https://ndstate.co1.qualtrics.com/jfe/form/SV_4UYppwHo4N18lAF
- Allow a minimum of three business days to be notified, via email, that the letter has been completed and is available for pick up at Ceres 302. (Please note that for the time being letters are only able to be sent via email)
