Submitting Verification Forms
There are a variety of ways you can submit your verification information. Type the information into the form, print, and sign prior to submitting; any unsigned forms will be returned to you for signature. You may submit the forms in the following ways:
Complete Forms Electronically Using DocuSign
All forms (except the Identity/Statement of Purpose Form) are available for completion using DocuSign.
READ BEFORE SUBMITTING FORMS:
The following instructions describe the sequence of steps required to complete the worksheet hosted in DocuSign™.
Sign-In Screen
This section must be completed by the student.
- Enter your name (middle name or initial is optional) and NDSU email address into the "NDSU Student" fields of the sign-in screen.
- Enter your Parent or Guardian's name and email address into the "Parent/Guardian" fields of the sign-in screen.
- Make sure to enter a parent email address that is frequently checked by your parent/guardian.
- Make sure the email address is typed correctly. Incorrect email addresses will cause the worksheet to not be processed by NDSU Financial Aid.
- Click the [BEGIN SIGNING] button at the bottom of the sign-in screen to continue.
Student Portion
This section must be completed by the student prior to sending it to the parent/guardian.
Please note: If you cannot complete the form in its entirety, you have the option to click the [FINISH LATER] button to email yourself a link to the unfinished form or you may click the Download or Print icon in the toolbar to save an electronic copy or print a paper copy of the form.
- Read and/or agree to the "Electronic Record and Signature Disclosure" statement if prompted.
- Close any open tool-tip bubbles and click on the [CONTINUE] button at the top of the form.
- Enter answers for required fields in Section I of the form (attach requested files if prompted).
- Use the yellow indicator tab on the left side of the form to navigate through the form if necessary.
- The attachments have a total file size limit of 25 MB. If your attachments exceed the limit, please contact ndsu.fa.verifi@ndsu.edu to receive instructions for a secure online file drop.

Please note: If a phone is your only method of capturing a document, using a free scanning app like "Microsoft Lens" (for Android or iOS) or "Adobe Scan" is preferred over taking a photo of the document.

- Proceed to Section V and click the [SIGN] button to add your DocuSign™ signature.
- If this is your first time using DocuSign, you may be asked to select and adopt a new signature template.
- If you have used DocuSign before, your previously-selected signature template will appear by default.
- If you need to make a change to the previously-selected signature, visit DocuSign.com and log in using your NDSU credentials to modify the settings on your existing account.
- Click the [FINISH] button at the bottom of the form to complete the student portion of the process.
- Any unanswered required fields will be highlighted in red when the [FINISH] button is clicked.
- Click the [FINISH] button again to submit the form after required fields have been populated.
- Your browser window will be redirected to the NDSU home page when finished.
- This indicates that the form has been forwarded to the parent.
- Let your parent know to watch for the email.
- You can expect to receive an email notification after NDSU Financial Aid receives and reviews the worksheet.
Parent/Guardian Portion
These sections must be completed by the parent once it arrives in their email.
Please note: If you cannot complete the form in its entirety, you have the option to click the [FINISH LATER] button to email yourself a link to the unfinished form or you may click the Download or Print icon in the toolbar to save an electronic copy or print a paper copy of the form.
- When the student portion of the form is completed an email notification (from "NDSU Financial Aid Verification via DocuSign") will be sent to the parent email address that the student provided in the sign-in screen.

Open the email notification and click the [REVIEW DOCUMENT] link in the message to open the form.
Please note: Note that the email notification could potentially be flagged as spam and might be delivered to a "Junk Mail", "Spam", or "Promotions" folder in the parent email account instead of the inbox.
- Read and/or agree to the "Electronic Record and Signature Disclosure" statement if prompted and click the [CONTINUE] button at the top of the form.
- Enter answers for required fields in Sections II, III, and IV of the form (attach requested files if prompted).

- Click the [FINISH] button again to submit the form after required fields have been populated.
- You will be asked if you want to save a copy of your document. Click DOWNLOAD, PRINT, or CLOSE.
- Clicking CLOSE will result in a final review of your document.
- Clicking the [CONTINUE] button will redirect your browser the NDSU Forms page when finished.
- You can expect to receive an email notification after NDSU Financial Aid receives and reviews the worksheet.
Notification of Completion
- After the parent clicks the [FINISH] button and the form is completed, NDSU Financial Aid will receive an email notification that your worksheet has been submitted.
- Financial Aid processors will open the email, review the worksheet, and enter it into their processing workflow.
- At the moment the worksheet is reviewed by Financial Aid, all parties (the student, the parent, and NDSU Financial Aid) will receive an email notification (from "NDSU Financial Aid Verification via DocuSign") that contains the following message: "All parties have completed NDSU Dependent Student Verification Worksheet for [Student Name]."
Information You May Need to Provide on this Form:
- Tax Return or Tax Return Transcript for previous tax year (or total of cash received during tax year) and Schedules 1, 2, and/or 3
- Dates related to Parent's marital status
- Names and ages of family members
- Names, ages, and amount of child support received
Scan or Photograph and submit online
Scan or photograph and submit forms online, submit forms using https://filetransfer.ndsu.edu/filedrop/ndsu.fa.verifi@ndsu.edu.
The Identity/Statement of Purpose Form and identification CANNOT be submitted electronically. That form and identification MUST be submitted either through regular mail (if signed by a notary) or dropped off in person at NDSU One Stop or Ceres Hall 205.
