Name Changes
A student's "name of record" at North Dakota State University is defined as the legal name under which the student was admitted to NDSU. "Legal name" is defined as that name verified by a birth certified date, marriage certificate, passport, or court order. Students wishing to change their name in their official academic record from the name of record under which they were admitted to the university must submit a Legal Name Change Request in eForms within Campus Connection and provide the legal documentation stating their new legal name.
Upon receipt and verification of this documentation, the registrar will change the student's name in the student's official academic record. "Official academic record" in this section is defined as the academic and biographical information of an individual student contained within NDSU's computerized student information system (Campus Connection). In addition, minor changes in the name (e.g., spelling corrections) may also be made. In such instances, the student may be expected to provide documentation such as a current driver's license with photo, passport, or resident alien card.
Preferred Name
Students can set a preferred name in Campus Connection. Instructions are found on the One Stop page.

Pronouns
Add your pronouns to your student record via Campus Connection.
- Log into Campus Connection
- Click on the Profile tile
- Under Personal Details, click on the Biographic tab
- Select your pronouns from the drop-down options
- Parents or guests who have been granted access to your student account will not see your personal pronouns when accessing your student account.
Personal Pronouns
When you confirmed your enrollment to the campus, some of your personal information, like your name, ID number and demographic information, was made available to campus employees who work with you, including staff, faculty and some student employees.
If you decide to share your personal pronouns, your pronouns will be included along with other information provided to campus employees.
Examples of where your personal pronouns will be available include the class roster provided to instructors and staff including your photo and the student record in campus systems that advisors and other student services staff use to work with you.
Sexual Orientation
Add your gender identity and/or sexual orientation to your student record via Campus Connection.

- Select your gender identity and/or sexual orientation from the drop-down options
- Parents or guests who have been granted access to your student account will not see your gender identity or sexual orientation when accessing your student account.
Gender Identity
Gender identity is not shared with class instructors or other campus partners, including health services. Authorized campus employees with access to your bio-demographic information will also have access to view your gender identity.

Gender Changes
A student's official academic record reflects the gender the student identified him- or herself as to the university when he or she applied for admission to NDSU. Students who subsequently wish to change the gender designation in their official academic record must provide the registrar with a "good faith" statement from the student to request a gender change. "Official academic record", in this section, is defined as the academic and biographical information of an individual student contained within NDSU's computerized student information system (Campus Connection).
Please note NDSU transcripts and diplomas do not specifically reference gender.
Campus Connection
Students will log into Campus Connection and click on the eForms tile as shown below to access the Legal Name Change Request Form. Click on Legal Name Change Request Form on the left side of the page. Students should monitor their NDSU email for any communication about the completion of the form.
