Agriculture Damage Assessment from June 2025 Storms

We're partnering with state agencies to assess agricultural impacts related to last week's severe weather. This data will be provided to state partners to support a federal disaster declaration and activate funds to aid in recovery efforts. If you were affected by the storms please fill out the assessment 
 
Things to note: 
• Please complete one assessment per individual/operation.
• No identifying information will be collected or shared.
• Data will be aggregated and reported at the county level.
